Patient Intake Form Application

Create a seperate application for patient intake forms that would allow the patient to fill out demographic and then parts of the initial evaluation electronically via a Tablet PC, touch screen, or kiosk.

    Yes patients will be able to electronically sign documents through the portal. We have a customizable set of policies (like HIPAA Consent, Financial Policy, etc.) that can be customized.

    Self check in isn't currently in the portal but we will be looking into that for a future version.

    Will patients have the ability to electronically sign intake forms with this as well? That would be helpful. Also, will patient self check in be available?

    Yep, you will be able to sync any questions in the initial evaluation you would like to ask the patient. Our goal is to make it so the patient could input the patient info / demographic, case info, and like the subjective portions of the initial evaluation via the portal and have it auto import.

    This would be so helpful with so many patients we see. Many of us have already seen this in other medical offices that we visit. We look forward to having this be part of our front desk process.

    I use IntakeQ (www.intakeq.com) and love it! It does exactly what all the comments below are referring to. It would be great to have this integrated with Theraoffice! Theraoffice, you will definitely be the leader in EMR's if you did this!

    This would would be fabulous. And with a patient portal so they could fill this out at home ahead of time, and it be directly imported to the file would be a great time saver. This could be a separate document which stays at the top of the patients chart separate from all other documents. The ability for a patient to review and update this periodically when they are repeats / returns on different episodes of care would be a great value as well.

    We do have developers actively working on this project, but I don't have any ETA yet unfortunately until it gets closer to testing. Sorry. This is one of our main goals for 2017 though.

    Filling out part of the subjective (chief complaint, pain, meds, etc) would be extremely helpful. We do pediatric and having developmental information (sitting, rolling, etc) entered by patient would be a huge time saver.

    In regards to the demographics, we use to print out a custom intake form with patients insurance and demographics pre-populated and all they had to due is verify their information. Every time there was an error in address or insurance (even date of birth) the patient just signed it verifying it was correct. No worries, though, the insurance company will let you know something is wrong when they deny your claim.

    Yes, we are planning on building out Function VIBE to be able to handle some intake features. The current plan is to be able to merge it with Documentation so a patient can fill out certain parts of the subjective parts of an eval and have it auto populate. This is where most of the manual entry of the eval can be sped up and is why we are targeting this part.

    From the feedback we've gathered, most people are still wanting someone at the front desk to populate the demographic and case info so the patient would just verify this from Function VIBE.

    Any ideas or suggestions?

    We don't have a time frame set yet just due to ICD-10 but we did have a prototype built.

    Really need this since we currently put info into our own forms, and then re-enter into TheraOffice. A few features that could help:

    * We all need our own forms, this needs to accommodate complete customization of what is captured

    * Maybe read in a multi-page PDF, and allow us to place fields into the form in the places we need them

    * Fields would be already available fields in the database, not custom fields

    * This would not replace the way information is currently input on patient setup. This would be a second way to capture the info and get it into the database

    * This also would then check for duplication of patients (as happens today when you create a new patient)

    * Would then allow for the printing of the completed form.

    * Would also need a button to re-print forms (since we make mistakes, insurances change, etc.)

    One data entry
    All gets into our T.O. database
    And we get our forms for intake.

    All in one step.

    Ryan, where can we find this update to implement??
    Thanks, we'll be releasing the Function Vibe app for iPad and Windows 8 soon (like in a few weeks). One idea that was presented was adding the intake entry part to this because it seemed like a good fit - you'd basically be able to give the patient the tablet after they got in and they would then do demographic and case info, then if Medicare they could perform their functional testing.

    The browser based web portal where patients could pre-register is the other direction which would allow the patient to enter demographic and case info through the web portal and then it would download into the inbound queue like a received fax.
